Night shift nurses definately qualifies. It's got quite a bit of scat content, lots of humiliation and torture as well.

Elfen lied, while not hentai, has quite a bit of very brutal violence. Lots of limbs being hacked off, decapitations, pens being jammed through someones skull. All done with telekenesis. Worth watching if you're a fan of guro.
