I've heard tons of "raped to death", but what are the actual cause(s) of death? Heart attack? Exhaustion (just how exactly does that work anyway?)

I don't think a medically and mentally "healthy" person could be raped to death, but there are definitely some health-risks involved, especially for non-healthy people.
One could be shocked or scared to death. A rapid heart rate increase could cause one to go into shock causing several vital function failures. But for this to happen, they either need a weak heart or an inclination to beat faster than necessary.
There's more, but I can't think of them right now. Also, I have no sources or proof of what I'm saying, but I do believe what I said.

When presented with certain death the mind and body simply shut down and accept fate, this has been widely documented not just in humans but in all animals. Of course it only occurs when you certain that you are going to die the very next moment, not in the same way you're sure this person attacking you is going to kill you if you.
And as far as I know you can't enter this state, remove the danger and keep the person in that mental state. I can't stop you from trying however.
I don't see a reason why simple physical exhaustion from a long enough rape session wouldn't be enough to kill someone. I was reading somewhere that certain aboriginal tribes in Australia literally chase animals until they drop, they only need weapons if the animal turns on them before that point. Also if you look at something like those MMO related deaths (was it last year?) if grinding on a game for 48 hours can kill a reasonably healthy adult I'm sure that a sustained sexual assault could have similar effects far more quickly along with internal bleeding and trauma.
